Job summary

21st Century Home Health Services in Northern California is hiring a Speech Therapist/Language Pathologist to join our field-based team serving Solano and surrounding counties. You will assess and treat speech, language, cognitive and swallowing disorders and collaborate with an interdisciplinary care team.

Qualifications

• CA Speech-Language Pathologist license is required.
• Current BLS/CPR certification.
• Valid CA driver license.
• Experience in home health care is preferred.
• Experience with high-acuity patients is a plus.

Responsibilities

• Provide skilled speech therapy services in patients' homes.
• Evaluate and treat speech, language, cognitive and swallowing disorders.
• Serve as educator and advocate within interdisciplinary care team.
• Collaborate with Case Manager and other clinicians to optimize outcomes.
