Job summary

• Multi-state licenses are preferred for broader practice.

Responsibilities

• Engage with patients during in-home health evaluations.
• Build trusted relationships to improve health outcomes.
• Collaborate with Signify Health clinicians and care teams.
• Travel locally within licensed states as needed; conduct virtual visits when required.
• Provide Diagnostic & Preventive Services as indicated by health plans.
