Job summary

Signify Health, part of CVS Health, is hiring a Part-Time Physician to perform in-home health evaluations. You will visit patients at their homes, review medical histories, conduct physical evaluations, and coordinate with health plans to ensure appropriate care.

Typically 29.5 hours per week, this role requires active medical licensure and willingness to travel within licensed states; you may conduct virtual visits as needed. You will use an iPad with a clinical workflow throughout evaluations.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Provide in-home health evaluations for plan members.
• Review medication history and perform physical evaluations.
