Job summary

Signify Health, part of CVS Health®, seeks a Part-Time Physician to provide in-home health evaluations, visiting patients in their homes and helping bridge gaps in care. You will perform history reviews and physical assessments while coordinating with health plans and care teams.

Visits last about 45 minutes, with no prescribing or lab orders required. You will use an iPad-based workflow and travel locally and within licensed states as needed.

Qualifications

• Active, unrestricted license in the coverage area.
• Multi-state licenses preferred.

Responsibilities
