Job summary

Signify Health, part of CVS Health, seeks a Part Time Physician to deliver in-home health evaluations. You will visit patients in their homes, conduct health assessments, and help bridge gaps in care using an iPad-based workflow.

Qualifications

• Active, unrestricted medical license in the coverage area.
• Multi-state licenses are preferred.

Responsibilities

• Provide in-home health evaluations, including history review and physical assessment.
• Perform diagnostic tests as ordered by the health plan when applicable.
• Maintain patient relationships and ensure access to appropriate care.
• Travel locally and within licensed states; conduct virtual visits as needed.
• Use iPad and clinical workflow for evaluations; no prescribing or changing treatments.
