Job summary

Signify Health, part of CVS Health, is seeking part-time physicians to provide in-home health evaluations. Clinicians will visit patients in their homes, review medical histories, perform physical assessments, and coordinate with health plans.

Travelling locally within licensed states is required; virtual visits may be needed. Active, unrestricted MD or DO licenses are required, with multi-state licenses preferred.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Perform in-home health evaluations with patients, including health history review and physical assessment.
• Utilize iPad-based workflow to document visits and coordinate with health plans.
• Travel locally within licensed states and to patient homes as needed.
• Conduct virtual visits when required.
