Job summary

Signify Health, part of CVS Health, is seeking a Part Time Physician to provide in-home health evaluations. You will connect with members in their homes, review medications and medical history, perform a brief physical evaluation, and support care coordination with health plans.

Travel locally and within licensed states is required, and you may conduct virtual visits as needed. Qualified MDs and DOs with active licenses in the coverage area are encouraged to apply.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses preferred.

Responsibilities

• Visit patients at home for comprehensive health evaluations.
• Review medication history and medical history during visits.
• Perform physical evaluations as part of in-home assessments.
• Coordinate with health plans to facilitate appropriate care referrals.
• Travel locally and within licensed states as needed.
• Conduct virtual visits when required.
