• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses preferred.

Responsabilités

• Conduct in-home health evaluations with patients, documenting medical history and physical findings.
• Perform diagnostic and preventive services as appropriate per health plan, without prescribing medication or altering current treatment.
• Engage patients and families to bridge gaps in care and connect to appropriate care teams.
