Job summary

Signify Health, part of CVS Health, is seeking a Part Time Physician to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

The visit lasts about 45 minutes and includes medication and medical history review, a physical evaluation, and optional diagnostic tests as ordered by the health plan. You’ll use an iPad and a clinical workflow for each evaluation; you are not required to prescribe medicines or alter current regimens.

Qualifications

• Provide In-Home Health Evaluations
• Excellent patient communication
• Ability to travel to patient homes

Responsibilities

• Provide in-home health evaluations to plan personalized care
• Review medication history and perform physical evaluations
