Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses preferred.
• Must maintain valid medical credentials.

Responsibilities

• Conduct in-home health evaluations with patients in their residences.
• Perform medication and medical history review and physical evaluation.
• Use Signify Health workflow and iPad for data collection.
• Coordinate care connections with health plans and PCPs as needed.
