Job summary

Responsibilities

• Provide case management and maintain a caseload of 7–10 participants.
• Complete intakes, assist with tenancy applications, and conduct weekly meetings.
• Record detailed notes and maintain program documentation in a timely manner.
• Coordinate with housing staff and community resources to support housing stability.
• Partner with participants to address barriers and support moving to independent housing.
