Job summary

Center City Housing Corp in Minnesota seeks a dedicated Case Manager to support tenants in a supportive housing building for adults with disabilities. You will participate in all aspects of case management, advocate for tenants, and collaborate with the Site Director to collect data and ensure services are accessed.

Qualifications

Degree in related field or equivalent experience with population.
Ability to read, analyze, and complete documentation required by government agencies; strong writing skills.

Responsibilities

Provide daily tenant advocacy and support to residents.
Identify and encourage goal completion; maintain progress notes and records.
Assist tenants to obtain health, social services, counseling, employment and benefits.
Teach or refer for independent living skills; collect tenant data with Site Director.
Maintain professional relationships with staff and community agencies.
