Job summary

Simpson Housing Services in Minneapolis, MN is seeking a dedicated case manager to support residents at Aeon’s OHP properties. You will work with a team delivering Harm Reduction and Housing First services to help residents gain stability in housing and community life.

You will maintain a caseload, conduct intakes, visit residents, connect them with benefits and community resources, and coordinate with property managers to address tenancy needs.

Qualifications

• Bachelor’s degree in human services or equivalent experience.
• Experience with housing resources and homelessness services.
• Driver’s License and access to a vehicle.

Responsibilities

• Maintain a caseload of participants and perform intakes.
• Conduct home visits and monthly participant meetings.
• Coordinate with housing resources and community services.
• Build relationships with property managers and support tenancy.
• Provide transportation and accompany residents to appointments.
