Job summary

Lutheran Social Service of Minnesota is seeking a Case Aide to support case management for families experiencing homelessness. You will link clients to community resources, provide crisis intervention, and help develop goals for stability and self-sufficiency.

Responsibilities include assisting with documentation, accompanying residents to view housing, and supporting funding reports and site visits. A valid driver’s license is required.

Qualifications

• Minimum of two years' experience in providing direct or indirect social services.
• Experience with homeless or low-income households is desired.
• Valid driver's license, acceptable driving record and a Motor Vehicle Check.
• Ability to communicate effectively with participants and professionals from diverse backgrounds.
• Ability to work independently and collaboratively with staff.

Responsibilities

• Assist case management personnel to provide case management services to families who are experiencing homelessness.
• Provide linkage to community resources and crisis intervention support for program participants.
• Accompany residents to view apartments and assist with needs assessment and information sharing.
• Maintain client files, records, reports and case notes; assist with funding reports and site visits.
