Job summary

Elite Hospice, part of LHC Group, is seeking a part-time Social Worker to provide medical social services under physician guidance and the Interdisciplinary Group. You will help patients and families understand social and emotional factors impacting health and support coping strategies.

Primary responsibilities include evaluating patient needs, contributing to a comprehensive Plan of Care, and guiding staff on community resources.

Qualifications

• Master's Degree in Social Work from an accredited program.
• Current CPR certification or ability to obtain within 90 days.
• Licensed Social Worker in the state of residence.

Responsibilities

• Assists patient and family to understand social factors related to health.
• Develops and contributes to an integrated Plan of Care daily.
• Instructs staff on community resources available to assist patients.
