Job summary

Elite Hospice is seeking a part-time Social Worker to provide medical social services under physician direction within a national post-acute care partner team. You will support patients and families, contribute to the Plan of Care, and connect patients with community resources.

You must hold a MSW, have a valid driver’s license and CPR certification (or acquire within 90 days). The role includes a comprehensive benefits package and opportunities for professional growth.

Qualifications

• Master's Degree in Social Work from an accredited program.
• Current CPR certification or ability to obtain within 90 days of hire.
• Licensed Social Worker in the state of residence or ability to obtain licensure.

Responsibilities

• Assist patients and care team to understand social and emotional factors affecting health.
• Contribute to development of a comprehensive Plan of Care on a daily basis.
• Instruct care team on available community resources and patient support services.
