Samaritan is seeking a compassionate Hospice Nurse for a part-time role serving patients in New Jersey. You will oversee the quality and expert care for assigned hospice patients, collaborating with the care team to ensure comfort and symptom management in home settings.

Requirements include NJ RN license, two years of nursing experience (BSN preferred); bilingual is a plus. Hourly rate ranges from $38.85 to $45.05, reflecting experience.

Qualifications

• Two years of nursing experience minimum.
• New Jersey RN license required; BSN preferred.
• Bilingual is a plus.

Responsabilités

• Perform patient assessments for comfort and relief of symptoms.
• Establish and implement an individualized plan of care for each patient.
• Assess the need for patient and family education and provide education as needed.
• Obtain appropriate physician orders to achieve symptom management.
• Triage and arrange home visit schedule, demonstrating efficiency in time and mile management.
• Make referrals to additional IDT members and collaborate with social workers related to their involvement.
