Job summary

Samaritan is seeking a Hospice Nurse for Camden/Gloucester/Atlantic County, full-time, on-site. The role includes overseeing patient care, coordinating symptom management, and collaborating with the care team. A NJ RN license is required; BSN preferred, and bilingual is a plus.

Qualifications

• Perform patient assessments for comfort and relief of symptoms.
• Establish and implement an individualized plan of care for each patient.
• Assess the need for patient and family education and provide education as needed.
• Obtain appropriate physician orders to achieve symptom management.
• Triage and arrange home visit schedule, demonstrating efficiency in time and mile management.
• Make referrals to additional IDT members and collaborate with social workers related to their involvement.
