Job summary

Samaritan is seeking a Hospice Nurse to oversee the quality and expert care of assigned hospice patients in the community. You will perform assessments, develop and implement plans of care, and provide education to patients and families.

The role requires at least one year of nursing experience in medical/surgical, oncology, geriatric, or hospice/home care settings, with BSN preferred; CHPN/OCN and BLS preferred; valid NJ driver's license and insurance; bilingual a plus.

Qualifications

• Minimum of one year nursing experience in medical/surgical, oncology, geriatric, or hospice/home care settings.
• Bilingual is a plus.
• BSN degree preferred.
• CHPN or OCN preferred; BLS training.

Responsibilities

• Perform patient assessments for comfort and symptom relief.

• Assess need for patient and family education and provide education as needed.
• Obtain physician orders to achieve symptom management.
• Triage and schedule home visits, managing time and mileage efficiently.
• Make referrals to IDT members and collaborate with social workers.

What You’ll Bring to Our Team:

• Minimum of one year nursing experience - medical/surgical, oncology, geriatric or related experience, hospice/home care experience preferred
• N. required; BSN Degree preferred
• CHPN or OCN preferred, BLS training
• NJ driver’s license and insurance coverage required
• Bilingual is a plus!
