Job summary

Soleo Health is seeking a Part-time Infusion Nurse to support infusion patients in Fort Collins, CO and surrounding area. You will provide direct patient care, assessments, care planning, and education for patients receiving infusion therapy in home or alternate sites, following care matrices and best practices.

Responsibilities include coordinating with care team, monitoring therapy, documenting encounters, and participating in on-call rotations.

Qualifications

• Current RN license in good standing with CO Board of Nursing.
• BSN preferred from an accredited program.
• Minimum 3 years clinical nursing experience; infusion therapy preferred.
• Experience with peripheral IV insertion and vascular access troubleshooting.

Responsibilities

• Provide direct patient care including initiation and maintenance of infusion therapy and education.
• Collaborate with pharmacists, prescribers, dietitians, home care staff and care partners.
• Evaluate home environment for safety and appropriateness of prescribed care.
• Develop and evaluate the plan of care and response to therapy with the team.
• Manage schedule via approved platform and communicate changes promptly.
• Document patient encounters and care activities per policy.
