Job summary

Providence at Home with Compassus is seeking a registered nurse to plan, implement, and evaluate home health care for patients in their residence, coordinating with physicians, families, and the care team. The role oversees an interdisciplinary team and ensures quality, compliant care.

The position requires an active RN license, current CPR, and a valid driver’s license with at least two years of RN experience in clinical care, preferably in home health.

Qualifications

• Nursing skills defined as generally accepted standards of practice.
• Good interpersonal skills.
• Two years of RN experience in a clinical care setting; home health preferred.
• Current, unencumbered State License as a registered Nurse.
• Current Driver’s License.
• Current CPR, negative TB screen and Hepatitis consent/declination.

Responsibilities

• Assesses home health patients to identify physical, psychosocial, and environmental needs.
• Case manages and provides clinical oversight to the interdisciplinary team and family.
• Implements and documents the plan of care and revises as necessary.
• Provides care with infection control measures and OSHA standards.
• Supervises HHA/LVN to ensure quality and continuity of services.
• Ensures continuity of quality patient care with proper documentation.
• Monitors cases for third party payor requirements.
• Performs on-call responsibilities as assigned.
• Communicates effectively with patients, families, team members, and physicians.
• Adheres to company policies and information systems security.
