Providence at Home with Compassus is seeking an experienced registered nurse to deliver skilled home health care in the Portland area. You will plan, implement, and supervise care with an interdisciplinary team, directing services at the patient’s home.

The role requires an active RN license, CPR certification, and two years of clinical RN experience, with a valid driver’s license and ability to manage complex patient needs. A comprehensive benefits package is offered.

Qualifications

• Graduate of an accredited college or university with RN licensure.
• Current, unrestricted state RN license and valid driver’s license.
• Current CPR certification and TB screening.
• Two years RN experience in clinical care; home health preferred.

Responsabilités

• Assess home health patients' physical, psychosocial, and environmental needs.
• Case manage and direct the interdisciplinary team and family.
• Implement and document the plan of care; revise as needed.
• Provide care with infection control to protect staff and patients.
• Supervise HHA/LVN to ensure quality and continuity of services.
• Ensure accurate documentation and payer compliance.
• Provide on-call coverage for patients and families as assigned.
• Communicate effectively with patient, family, and health professionals.
