Job summary

Providence at Home with Compassus is seeking a dedicated Home Health Registered Nurse to provide expert care in patients’ homes. You will conduct comprehensive assessments, develop and implement individualized care plans, and deliver outcome-driven interventions under the physician-ordered Plan of Care.

You will work with an interdisciplinary team to support patient and family goals, while maintaining high safety and quality standards in a dynamic home health environment.

Qualifications

• Current RN license in state of employment (unencumbered).
• BLS certification (American Heart Association).
• Experience in home health, acute care, or skilled nursing facility preferred.

Responsibilities

• Conduct comprehensive patient assessments upon admission and during care.
• Develop and implement individualized plans of care with a physician-ordered Plan of Care.
• Collaborate with interprofessional team, patient, and family to ensure high-quality care.
• Document care activities accurately and timely using electronic health records.
