Job summary

Crossroads Home Health & Hospice in Vallejo, CA is seeking a part-time Home Health Physical Therapist to cover Vallejo and Solano County. You will conduct in-home PT evaluations, implement home exercise programs, and coordinate with the RN case manager to ensure smooth discharges.

Requirements include an active CA PT license, home health experience, CPR, a reliable vehicle, and the ability to pass a background check. Bilingual English/Spanish is a plus.

Qualifications

• Must hold an active California Physical Therapist license.
• Home health experience preferred; SNF/outpatient acceptable.
• Current CPR certification required.
• Reliable vehicle, valid driver’s license and auto insurance.
• Ability to pass a background check.
• Bilingual English and Spanish a plus, not required.

Responsibilities

• PT evaluations and home visits.
• OASIS collaboration on therapy-only cases.
• Home exercise programs and caregiver training.
• Discharge planning with RN case manager.
• Document in the EMR within 48 hours of each visit.
