Job summary

Crossroads Home Health & Hospice – Vallejo is seeking a part-time Home Health Physical Therapist to serve Marin County. You will evaluate and treat patients in their homes, own your schedule, and coordinate with a clinical supervisor. Travel is required within Marin communities; documentation in the EMR within 48 hours is expected.

California PT license, CPR, and reliable vehicle are required. Home health experience preferred; bilingual English/Spanish a plus but not required.

Qualifications

• Active California Physical Therapist license
• Home health experience preferred; SNF/outpatient PTs oriented
• Current CPR certification
• Reliable vehicle, valid driver’s license and auto insurance
• Ability to pass a background check
• Bilingual English and Spanish a plus, not required

Responsibilities

• PT evaluations and treatment visits in the home
• OASIS collaboration on therapy-only cases
• Home exercise programs and caregiver training
• Discharge planning and collaboration with the RN case manager
• Document in the EMR within 48 hours of each visit
