Job summary

ABL Health Care in Vallejo, CA is looking for a full-time Physical Therapist (PT) to join our Home Health team. Responsibilities include evaluating patients, creating care plans, and delivering skilled intermittent care in patients' homes, with support from clinical leadership.

The ideal candidate holds a California PT license, has 1 year of Home Health/hospice experience, a valid CA driver's license, and can travel 15–25 miles regularly.

Qualifications

• California PT license required.
• 1 year home health or hospice experience.
• Current CA driver's license.
• Ability to travel 15–25 miles regularly.

Responsibilities

• Provide, coordinate and oversee skilled intermittent care to patients and include instruction and education of patients and/or caregivers to promote maximal functional status in the patient’s home.
• Regularly evaluate and re‑evaluate patient status.
• Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or as needed Director of Nursing.
• Initiate, develop, implement and revise the patient’s plan of care (POC).
• Address all problems identified in the POC.
• Provide educational information to the patient/family/caregiver concerning knowledge deficits or needs regarding the patient’s condition.
• Communicate with other disciplines involved in the case as warranted.
• Maintain accurate clinical records and submit documentation according to company policy using a company‑provided device.
• Comply with all company policies and procedures to ensure high standards of care.
