Job summary

Crossroads Home Health & Hospice in Vallejo, CA is seeking a part-time Home Health Physical Therapist to cover Vallejo and Solano County. You will conduct in-home PT evaluations, implement home exercise programs, and coordinate with the RN case manager to ensure smooth discharges.

Requirements include an active CA PT license, home health experience, CPR, a reliable vehicle, and the ability to pass a background check. Bilingual English/Spanish is a plus.

Qualifications

Home Health Physical Therapist (Part-Time) | Vallejo & Solano County

Solano County home health PT, part-time. Small local team, a clinical supervisor who returns your calls, and a territory covering Vallejo, Benicia, American Canyon and Fairfield that you can drive in a morning. Ideal for a PT with outpatient or SNF experience who wants the autonomy of home health without a giant service area.

Pay and Benefits

• $50 to $60 per hour, based on experience
• Mileage reimbursed at $0.76 per mile

What You Will Do

• PT evaluations and treatment visits in the home
• OASIS collaboration on therapy-only cases

What We Are Looking For

• Active California Physical Therapist license
