Job summary

BAYADA Home Health Care is seeking an experienced Physical Therapist for a per diem opportunity providing home health visits in Montgomery, Maryland. The role covers adult and geriatric clients across Allegany County and neighboring territories.

As a PT, you will join a multidisciplinary team delivering rehabilitative care in the home setting, with emphasis on wound care, mobility, and functional independence. BAYADA emphasizes employee development and comprehensive benefits.

Qualifications

• A current MD Physical Therapist - PT license.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organization, and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements including mobility, gait, strength, balance, and safety assessments.
• Develop and implement individualized care plans with manual therapy, gait training, and balance interventions.
• Continually assess and revise the PT care plan and collaborate with nursing on the multidisciplinary plan.
• Educate clients and family members on rehabilitative care to promote health and independence.
• Accurately document observations and interventions using a tablet-based EMR system.
