Md-Home-Health-Md-Home-Assis seeks an Intake/Staffing Coordinator to lead patient referrals and intake operations, ensuring admissions comply with state, federal, and Joint Commission standards. You will collaborate with clinical staff, referral sources, and marketing to deliver efficient, compliant care while maximizing reimbursements.

Responsibilities include screening referrals, securing authorizations, coordinating with staff, and maintaining accurate records while upholding confidentiality

Qualifications

• Minimum of one year of admissions experience in home health, SNF, hospital discharge, or case management.
• Knowledgeable of medical practices, terminology, laws and guidelines related to home health.
• Able to relate positively and cooperatively with physicians, associates and external agencies.
• Capable of thriving in a fast-paced, friendly and professional environment.
• Strong written and verbal communication; good negotiation and public relations skills.

Responsabilités

• Direct daily referral and intake processes, ensuring regulatory and organizational policy adherence.
• Screen and evaluate referrals to confirm eligibility for home health services.
• Maintain relationships with referral sources and promptly process inquiries and admission decisions.
• Verify primary insurance coverage, secure authorizations, and document for reimbursement.
• Coordinate with home health staff to provide accurate information for initial visits.
• Identify opportunities to improve services and workflows for better outcomes.
• Understand company contracts and community resources to redirect patients when appropriate.
• Communicate effectively with all members of the care team to support coordinated care.
• Uphold professionalism, patient confidentiality, and ongoing education.
