Qualifications

• Medical Assisting Certification (CMA) preferred.
• At least 1 year of experience in a health-related field.

Responsibilities

• Conducts an initial health risk and needs assessment.
• Provide information and guidance to patients, providers, and caregivers.
• Assist with plan of care activities and coordination with care team.
• Schedule patients and coordinate transportation as needed.
