Job summary

Christ Community Health Services in Memphis seeks a proactive Preventative Care Engagement Specialist to strengthen population health within our PCMH initiatives. You will identify care gaps and drive outreach to boost preventive services and screenings across the patient population.

The role requires CMA credentials, 1–2 years healthcare experience, and proficiency with Word and Excel. Some travel (5–30%) may be needed to coordinate care with clinics and partners in Memphis.

Qualifications

• CMA credential or certification required/preferred.
• Experience in healthcare settings serving underserved populations preferred.
• Proficiency with basic medical terminology and data reporting.

Responsibilities

• Identify and stratify patients based on risk and quality metrics.
• Drive targeted outreach to close care gaps and coordinate preventive services.
• Document interactions accurately in EHR and related systems.
• Collaborate with clinical teams to ensure timely preventive screenings.
