Job summary

Privia Health is seeking a Health Advisor to provide care coordination support for at-risk patients. You will perform inbound/outbound calls to assist in filling gaps in care and improving patient outcomes across in-person and virtual settings.

The role requires experience with electronic medical records, HIPAA compliance, and strong communication skills. A hybrid schedule (3 days remote, 2 days in-office) is offered with a focus on quality patient care.

Qualifications

• Experience with care coordination and patient outreach.

Responsibilities

• Assist physician practices in identifying and completing quality care goals through direct patient outreach.
• Collaborate with care team to support care coordination for patients.
• Review patient non-compliance lists and conduct telephonic outreach to ensure timely visits.
• Answer and resolve business office inquiries related to population health or care coordination.
• Motivate patients to be proactive about their healthcare.
• Assist patients in accessing care at the appropriate source.
• Develop relationships with care coordination teams and coordinate home care referrals.
• Schedule PCP or specialist appointments and route clinical calls to the appropriate resource.
• Ensure HIPAA compliance and handle other duties as assigned.
