Position Summary:

The Quality Engagement Advisor I specializes in conducting initial health risk assessments for patients and providing information/support to patients when needed. In this role, the Quality Engagement Advisor provides information and guidance to patients, providers, and caregivers through inbound and outbound calls. In addition, the Quality Engagement Advisor is an essential part of the Care Management and Behavioral Health teams in assisting with assigning patients to their Care Coordinators.

Key Results Areas (KRAs):
  • Conducts an initial health risk and needs assessment.
  • Serve as a resource by assisting with plan of care activities and providing direction to patients
  • Guides patient toward maintenance of contact regularly to interdisciplinary care team members
  • Acts in accordance with laws and procedures regarding patient confidentiality and release of information, Americans with Disabilities Act, and other laws protecting rights
  • Participates in the development, implementation, evaluation, and ongoing revision of initiatives to improve quality, continuity, and cost effectiveness of care
  • Works collaboratively with other departments and services to define and investigate areas of inefficiency and participates in process improvement initiatives.
  • Ensures consistent data capture to identify trends/problems related to the delivery of care and potentially avoidable hospital readmissions.
  • Assist with patient scheduling and transportation needs
  • Maintains strict confidentiality of client, company, and personnel information

Requirements
Education & Experience:
  • A minimum of a high school degree is required; Medical Assisting Certification (CMA) preferred
  • At least 1 year of experience working in a health-related field
  • Experience working with Medicaid population preferred
  • Home/Community-health-based telecommunication experience desirable.
  • Excellent interpersonal and analytic skills required to accomplish assessment calls and coordination/prioritization of interventions
