Job summary

Whole Life is seeking compassionate Direct Support Professionals to join our Brooklyn, CT team for a part-time role focused on person-centered care for adults with developmental and intellectual disabilities. You’ll help with daily living activities and support community integration.

You will accompany clients to appointments, assist with meals and routines, monitor health, and document observations while collaborating with managers and families to implement care plans.

Qualifications

• High School diploma or GED required.
• Valid driver’s license and identity documents required.
• Able to pass background checks and health screenings within 30 days of hire.
• Must be able to perform physical activities including lifting up to 100 lbs.
• Ability to document care in English and communicate clearly with clients and staff.

Responsibilities

• Assist with Activities of Daily Living (ADLs): bathing, dressing, grooming, toileting, and meal preparation.
• Administer medications, monitor vital signs, support medical appointments.
• Maintain a clean, safe living environment through light housekeeping and laundry.
• Engage clients in social activities, community outings, and promote independence.
• Use mobility equipment (e.g., Hoyer lifts) with proper technique.
• Document daily activities, observations, and incidents accurately.
• Maintain confidentiality per HIPAA.
