Providence Medford- OBGYN Health Center and Providence Medical Group Stewart Meadows seeks a part-time Diabetes Educator to educate clients with diabetes of all ages. Duties include health and education assessment, planning and evaluating education plans, and collaborating with RNs and RDs in classroom or outpatient settings.

The role emphasizes patient-centered care, language access, and community outreach as part of professional practice.

Formación

• Oregon Dietitian License upon hire.
• National CDR Registered Dietitian upon hire.
• Three years of diabetes education experience within the last 2 years.
• Experience applying adult learning theory to diabetes education.
• Experience with clinical diabetes management.

Responsabilidades

• Provide education for clients with diabetes of all ages in classroom or outpatient settings.
• Develop and implement an education plan based on assessment and collaboration with RNs & RDs.
• Engage in professional development and community outreach as part of practice.
