Life Steps Foundation Southern California Adult Services (LSF SoCal Adult) is seeking a compassionate Community Support Facilitator for the Buena Park and La Habra area on a part-time basis. The role focuses on direct client care, daily living assistance, and coordinating services to promote independence.

Qualifications

• High School Diploma or GED required.
• CNA certification preferred.
• Experience in case management or as a personal attendant preferred.
• Reliable transportation required.
• Valid Driver's License and auto insurance required.
• Bilingual skills preferred.

Responsibilities

• Provide personal care services to clients, including meal preparation, assistance with eating, bathing, toileting, and supporting general physical activity.
• Perform light cleaning duties to maintain a safe and comfortable environment for clients.
• Assist with medication management by monitoring and supporting clients as needed.
• Offer on-call assistance to clients in the event of illness or emergency situations.
• Support individuals with mobility needs, including assistance with wheelchairs and other mobility devices.
• Help clients identify and utilize natural and community supports to enhance their independence.
• Encourage and assist individuals in developing and maintaining meaningful relationships.
• Assist clients in finding suitable roommates and housing options as needed.
• Accurately complete and submit billing and payroll documentation in a timely manner.
• Follow the approved monthly schedule and ensure completion of authorized service hours.
