Job summary

The Independent Living Program is seeking a Community Support Facilitator to assist adults with developmental disabilities in living independently. You will help with daily living skills, budgeting, and community activities, while coordinating transportation and appointments.

Requires a high school diploma or GED, with at least 1 year in social services preferred. Driving with mileage reimbursement is required, and CPR/First Aid certification is part of the training path.

Qualifications

• High School Diploma or GED required with preferred social services experience.
• Experience supporting adults with developmental disabilities is desirable.
• willingness to drive clients using personal vehicle with mileage reimbursement

Responsibilities

• Support individuals living independently with daily living skills and community access.
• Assist in developing Personal Support Plans and coordinate services like transportation.
• Maintain documentation, submit timesheets, and attend trainings and meetings.
• Communicate promptly with supervisors and the client’s support network.
