Life Steps Foundation Southern California Adult Services (LSF SoCal Adult) seeks a compassionate Community Support Facilitator for a part-time position in the Buena Park and La Habra area. The role focuses on helping adults with disabilities maintain independence and quality of life through personal care, mobility support, and community connection.

The ideal candidate has a High School Diploma or GED, CNA certification is a plus, and prior experience in case management or as a personal

Qualifications

High School Diploma or GED required.
CNA certification preferred.
Experience in case management or as a personal attendant preferred.

Responsibilities

Provide personal care services (meals, eating assistance, bathing, toileting, mobility support).
Perform light cleaning to maintain a safe environment.
Assist with medication management and monitoring.
Provide on-call assistance for illness or emergencies.
Support mobility and wheelchair use as needed.
Help clients access natural and community supports for independence.
Encourage development of meaningful relationships.
Assist clients in finding housing options or roommates if needed.
Complete billing and payroll documentation timely.
Follow monthly service hours schedule and ensure tasks are completed.
