Life Steps Foundation Southern California Adult Services (LSF SoCal Adult) is a dedicated organization serving over 300 participants throughout Southern California. Our mission is to support adults with disability challenges, helping them maximize independence and quality of life.

We are seeking a compassionate Community Support Facilitator to join our team on a part-time basis in the San Gabriel Valley area.

Qualifications

• High School Diploma or GED is required.
• Experience in case management or as a personal attendant preferred.

Responsibilities

• Provide personal care services including meals, eating assistance, bathing, toileting and mobility.
• Perform light cleaning duties as necessary.
• Monitor and assist with medication management.
• Provide on-call assistance to clients during illness or emergencies.
• Assist individuals with wheelchairs and mobility needs.
• Identify community supports and assist in utilizing them.
• Support clients in developing and maintaining relationships.
• Assist in finding suitable housing and roommates.
• Submit billing and payroll documents accurately and on time.
• Adhere to the approved monthly schedule and complete authorized service hours.

We are currently seeking a compassionate and dedicated Community Support Facilitator to join our team on a part-time basis in the San Gabriel Valley area.

Responsibilities

• Provide personal care services to clients, including meal preparation, assistance with eating, bathing, toileting, and general physical activity.
• Perform light cleaning duties as necessary.
• Monitor and assist with medication management.
• Provide on- call assistance to clients in the event of an illness or emergency.
• Assist individuals with wheelchair and other mobility needs.
• Identify natural and community supports and assist the individual in using them.
• Support individuals in developing and maintaining relationships.
• Assist in finding suitable roommates and housing.
• Submit accurate, complete, and legible billing and payroll documents in a timely manner.
• Adhere to approved monthly schedule and complete authorized service hours.

Qualifications

• High School Diploma or GED required.
• Prior experience in case management or as a personal attendant.
• Reliable transportation is a must.
• Valid Driver's License and auto insurance.
