Responsibilities

• Provide personal care services (meals, eating assistance, bathing, toileting, mobility support).
• Perform light cleaning to maintain a safe environment.
• Assist with medication management and monitoring.
• Provide on-call assistance for illness or emergencies.
• Support mobility and wheelchair use as needed.
• Help clients access natural and community supports for independence.
• Encourage development of meaningful relationships.
• Assist clients in finding housing options or roommates if needed.
• Complete billing and payroll documentation timely.
• Follow monthly service hours schedule and ensure tasks are completed.

Life Steps Foundation Southern California Adult Services (LSF SoCal Adult) is a mission-driven organization dedicated to empowering adults with disabilities throughout Southern California. Serving over 300 participants, LSF SoCal Adult is committed to helping individuals maximize their independence, honor their personal choices, and maintain a high quality of life. The organization is seeking a compassionate and reliable Community Support Facilitator to join the team on a part-time basis in the Buena Park & La Habra area. This role is ideal for individuals who are passionate about making a positive impact in the lives of others and who thrive in a supportive, community-focused environment.

Responsibilities

• Provide personal care services to clients, including meal preparation, assistance with eating, bathing, toileting, and supporting general physical activity.
• Perform light cleaning duties to maintain a safe and comfortable environment for clients.
• Assist with medication management by monitoring and supporting clients as needed.
• Offer on-call assistance to clients in the event of illness or emergency situations.
• Support individuals with mobility needs, including assistance with wheelchairs and other mobility devices.
• Help clients identify and utilize natural and community supports to enhance their independence.
• Encourage and assist individuals in developing and maintaining meaningful relationships.
• Assist clients in finding suitable roommates and housing options as needed.
• Accurately complete and submit billing and payroll documentation in a timely manner.
• Follow the approved monthly schedule and ensure completion of authorized service hours.

Qualifications

• Reliable transportation required
• Valid Driver's License and auto insurance required
• Bilingual skills preferred
