Job summary

Imagine the Possibilities, Inc. is seeking a part-time, on-site Community Integration Professional in Iowa (Day Habilitation setting).

You will collaborate with individuals served to identify interests and goals, building meaningful daily schedules that foster community participation and skill development. You will provide coaching to help individuals achieve goals, follow behavior care plans, and document progress while ensuring safety and advocacy for independence.

Qualifications

• Mission-oriented and empowerment focus.
• Adaptable in caregiving, eager to learn.
• Strong communication with team and individuals served.
• Able to advocate while following leadership.
• Must be at least 18 years old.
• Must have driver’s license and reliable transportation.
• Provide proof of $100,000/$300,000/$50,000 auto liability coverage.
• High school diploma or equivalent preferred; not required to have higher education.
• Must be able to work in the U.S. without sponsorship.

Responsibilities

• Collaborate with individuals to identify interests, strengths, and goals, and develop meaningful daily and weekly schedules that include community participation, hobbies, volunteer work, and cultural, civic, or religious activities.
• Provide direct support by teaching individuals how to complete tasks and achieve goals independently, using coaching and instruction.
• Follow individualized behavioral care plans and support positive behavior.
• Maintain accurate progress documentation for continuity of care and accountability.
• Provide a safe, supportive environment and advocate for independence and person-directed decisions.
