Job summary

Imagine the Possibilities, Inc. seeks a compassionate Community Integration Professional for a part-time, on-site role in Boone, IA. The Day Habilitation setting focuses on community inclusion and skill development, with coaching rather than doing tasks for individuals served.

You’ll collaborate with teammates, families, and individuals to design daily schedules, support goal achievement, document progress, and advocate for independence while maintaining safety and person-centered care.

Qualifications

• Must be able to communicate effectively with team members, individuals served, and families.
• Demonstrated reliability and willingness to learn in a caregiving setting.
• Able to balance compassion with boundaries and safety protocols.
• Age 18+ and valid driver’s license are required.
• Reliable transportation to support community-based activities.

Responsibilities

• Collaborate with individuals to identify interests, strengths, and goals.
• Develop daily and weekly schedules with community activities and hobbies.
• Provide coaching to develop skills and achieve goals, rather than completing tasks for someone.
• Follow behavioral care plans and support positive participation.
• Document progress and outcomes accurately to ensure continuity of care.
• Advocate for independence, safety, and person-directed decision-making.
