You will collaborate with the care team, document interventions in Setworks, manage daily living tasks, and adhere to the six rights for medications, with comprehensive training provided.

Qualifications

• Age requirements vary by setting: 16+ for SCL, 18+ for HAB.

Responsibilities

• Goal Input & Person-Centered Support: Develop individualized goals and provide direct support to promote independence and participation in activities.
• Behavioral & Rights-Based Support: Use approved strategies while respecting rights per the interdisciplinary team.
• Documentation & Compliance: Complete Iowa Code–compliant documentation in Setworks daily.
• Direct Care & Community Integration: Provide hands-on support in home and community settings, including daily living assistance and transportation.
• Medication, Scheduling & Professional Responsibility: Administer and document medications per the six rights and maintain attendance.
