Imagine the Possibilities, Inc. is seeking a dedicated Community Integration Professional to support individuals in Day Habilitation and Supported Employment settings.

You will collaborate with staff, individuals served, and families to craft individualized plans promoting inclusion, skill development, and meaningful community participation. Responsibilities include coaching, job exploration, and safety-focused behavioral support, with emphasis on person-centered outcomes and documentation.

Qualifications

• Willingness to learn in a caregiving field; adaptability.
• Strong communication with team and individuals served.
• Confident advocate balancing boldness and humility.
• Must be at least 18 years old; valid driver's license.
• Reliable transportation with evident insurance coverage.
• Education: high school diploma or equivalent; US work authorized.

Responsabilités

• Collaborate with individuals to identify interests, strengths, and goals while promoting independence and community participation.
• Provide direct support, coaching, and instruction to develop life, social, and vocational skills.
• Assist individuals with employment exploration, applications, interviews, and workplace readiness.
• Implement behavioral support strategies while promoting safety, dignity, and self-determination.
• Maintain accurate documentation of progress, services, and incidents; participate in training and team activities.
