Job summary

Melwood's Community Services Division in Upper Marlboro, MD seeks a Community Connector for a 30-hour week, Monday–Friday, 9:00 AM–3:00 PM. The role supports individuals with disabilities in a community-based day services model, promoting independence and personalized growth through meaningful activities.

Responsibilities include implementing Individual Plans, coordinating with the interdisciplinary team, and documenting progress.

Qualifications

• H.S. Diploma or equivalent required with three years' experience in disability support.
• One year demonstrated ability providing support in a respectful manner.
• Bachelor's degree in special education, therapeutic recreation, psychology or related field preferred.
• Driver's license is required and vehicle used to transport persons served.

Responsibilities

• Work with assigned individuals to implement their Individual Plan and promote community integration.
• Engage individuals in meaningful community activities to develop skills and social supports.
• Provide services outlined in Individual Plans and ensure progress benchmarks are met.
• Complete progress notes, daily logs, timesheets and related documentation thoroughly and promptly.
• Develop activities that promote community, self-help employment, and independent living.
