Job summary

Melwood Inc. in Bowie, MD, seeks a Community Connector to support individuals in a community-based day services model. Schedule: Wednesday, Thursday and Friday, 9:00 AM–3:00 PM, totaling 18 hours per week, on-site at Bowie, MD.

The role focuses on promoting independence and meaningful daily activities aligned with each person’s desires, strengths and needs. Responsibilities include implementing Individual Plans, enabling community integration, documenting progress in daily logs and notes, and

Qualifications

• HS diploma or equivalent required.
• Three years of experience with people with disabilities.
• Vehicle use for transporting individuals served.
• Bachelor's degree preferred.

Responsibilities

• Work with assigned individuals to implement their IP and promote community integration.
• Engage individuals in meaningful community activities to build skills.
• Complete progress notes and logs accurately and promptly.
• Attend required trainings and team meetings.
• Other duties as assigned.
