Job summary

Melwood in Maryland seeks a Community Connector to support individuals in a community-based day services model. You will help implement Individual Plans, promote independence, and plan person-centered daily activities designed around each person’s goals.

The role emphasizes meaningful community engagement, skill development, and respectful communication. You will document progress, participate in trainings, and coordinate with staff to ensure timely, client-focused services.

Qualifications

• High School diploma or equivalent required.
• Bachelor's degree in special education, therapeutic recreation, psychology, or related field preferred.

Responsibilities

• Work with assigned individuals to implement their IP, integrate them into the community, build skills, and develop individualized daily schedules.
• Engage person served in meaningful community activities to develop skills and social supports.
• Provide services as outlined in Individual Plans, ensuring goals are appropriate and progress benchmarks are met.
• Demonstrate positive behavioral management and communicate in a person-centered, respectful manner.
• Complete progress notes, daily logs, timesheets, mileage logs, and other documentation thoroughly and promptly.
• Facilitate an individual's self-esteem through positive interactions and building trust.
• Develop activities that promote community participation, self-help employment, and independent living.
• Communicate with the supervisor and team about schedule implementation.
• Attend required trainings, staff meetings, stakeholder meetings, and supervision meetings.
• Other duties as assigned.
