Job summary

Melwood seeks a Community Connector to support individuals in community-based day services in Upper Marlboro, MD. You will help integrate clients into the community, build skills, and create personalized daily schedules aligned with each person’s goals.

The role requires a high school diploma or equivalent, 3+ years serving people with disabilities, a valid driver's license, and the ability to transport clients.

Qualifications

• H.S. Diploma or equivalent required and three years experience working with people with disabilities or other behavioral issues.
• One year demonstrated ability in providing support in a respectful manner that meets the various needs of individuals served.
• A driver's license is required and Melwood transportation policies must be followed.

Responsibilities

• Work with assigned individuals to implement their IP, integrate them into the community, build skills, and develop individualized goal-orientated daily schedules.
• Engage person served in meaningful community activities that assist people in developing the skills and social supports necessary to promote positive personal growth.
• Provide services as outlined in Individual Plans - ensuring goals developed are appropriate, implemented, and progress benchmarks are being met.
• Demonstrate positive behavioral management techniques and communicates with individuals in a person-centered, age-appropriate manner.
• Complete progress notes, daily logs, timesheets, mileage logs, individual planning activities, and other documentation thoroughly, accurately, and promptly.
• Facilitate an individual's self-esteem through positive interactions and building a relationship of trust.
• Develop activities that promote community, self-help employment, and independent living.
• Communicate with the supervisor and people supported regarding schedule implementation.
• Attend and actively participate in required training, staff meetings, stakeholder team meetings, and supervision meetings.
• Other duties as assigned.
