Qualifications

• Maintain DFS certification.

• Ability to perform physical tasks and lift up to 50 lbs.

Responsibilities

• Provide direct patient care as assigned.
• Provide personal hygiene care and daily living activities.
• Assist with transfers, repositioning and ambulation.
• Provide basic nursing care under supervision of a RN/therapist.
• Prepare meals respecting client dietary needs.
• Provide companionship and engage in activities as needed.
• Light housekeeping duties as assigned.
• Maintain accurate records as required by the agency.
• Report changes in client condition promptly.
• Follow Plan of Care and EVV timekeeping requirements.
