Responsibilities

• Provide direct patient care as assigned in home settings
• Assist with personal hygiene, meals, and daily living activities
• Assist with transfers, repositioning, and ambulation as directed
• Provide basic nursing tasks under supervision per care plan
• Maintain accurate care records and EVV time reporting
• Communicate changes or concerns to the supervising team
• Support independence within client abilities and safety guidelines
• Uphold patient privacy and confidentiality
